Rates include the chauffeur, fuel and standard gratuity; weekend evenings carry a 3–4 hour minimum. For fixed-route trips such as airport transfers, a flat rate is usually cheaper than hourly.
Billing starts at your scheduled pickup time and ends at the final drop-off. Waiting time while you're at dinner or an event is part of the booked hours, so the chauffeur stays with you. If your night runs long, additional time is billed in 30-minute increments at the same hourly rate — never a penalty rate.

Yes, on weekdays a 1-hour minimum applies to sedans, SUVs and most stretch limos. Weekend evenings require a 3-hour minimum because of demand.
